Beginning summer 2026, all international students must submit a video statement and report card to demonstrate English proficiency. In addition, families must also submit the Authorization for Medical Treatment and Tuberculosis (TB) by the noted deadlines.
1. English Language Requirement
International students must be proficient in English and able to demonstrate their ability to understand and communicate (i.e.; read, write, speak) in English at their appropriate grade level. This will ensure the safety of each student and enable your child to have a positive summer experience. We are unable to provide accommodations for English language learners, and the school reserves the right to modify schedules or remove students from courses if it deemed appropriate.
Beginning in Summer 2026, all international students, returning or attending Punahou Summer Programs for the first time, must submit a Student Video Statement (grades K – 12) and a Report Card (grades 1 – 12) as outlined in the document below.
Families must receive approval to register prior to registering on Friday, March 6, 2026.
2. Authorization for Medical Treatment FormParents or guardians of a visiting student(s) will be required to download, sign and upload the completed Authorization for Medical Treatment form annually. Signing this form will give permission to our Health Center staff to provide medical treatment to your child in case of any illness or injury. April 25, 2026.
3. Tuberculosis (TB) Form
Hawai‘i State Department of Health Requirements for Schools
Students who are not currently enrolled in a Hawai‘i school (which includes home-schooled students, International and continental U.S. residents) must meet the State of Hawai‘i assessment requirements and provide a TB clearance that has been done within 12 months prior to the start of their Summer class or lesson.
Students who have a TB form on file with Punahou School that was submitted on or after Summer 2022 do not need to submit a new form.
All applicants must have a U.S. licensed practitioner (MD, DO, APRN, PA) complete and sign the Tuberculosis form to confirm that the student is free from TB. The Hawai‘i Department of Health (DOH) Form will also be accepted.
The following are acceptable forms to TB screens or tests.
- Negative TB Risk Assessment – This assessment must be completed and signed by a U.S. licensed practitioner.
- Negative Tuberculin Skin Test (TST) – This test must be performed in the U.S. or on U.S. soil. Results must be interpreted and signed by a U.S. licensed practitioner (MD, DO, APRN, PA).
- QuantiFERON® TB Gold-in-Tube Test or T-Spot® Test – This test must be interpreted and signed by a U.S. licensed practitioner.
- Positive Test and Negative Chest X-ray. The chest x-ray must be interpreted and signed by a U.S. licensed practitioner (MD, DO, APRN,PA)
Tuberculosis (TB) forms must be submitted by the following deadlines.
Students will not be allowed to attend summer classes or lessons until these mandatory health forms have been received and approved by the Health Center Office.
Medical/Travel Insurance We highly recommend that International and Continental U.S. students have medical and/or travel insurance for the safety of their children and to ensure proper medical care.
